2. Balance and key employee evidence

  • Recordkeeper balance detail as of the determination valuation date
  • Key employee listing with methodology notes and approver
  • Forfeiture, reallocation, and distribution activity reconciled through the valuation date
  • Related-party or controlled-group notes documented when applicable (coordination only—substance with counsel)

3. Determination and minimum contribution

  • Official top-heavy determination output stored with run ID and balance file hash
  • Top-heavy ratio and pass/fail summary cited from the vendor report (not email summaries alone)
  • Minimum contribution calculation package attached when plan is top-heavy
  • Funding source and payroll trust confirmation tracked to completion
